First, slide the O ring over the outside threaded part (where a cap would go) to help form an air seal (conversely, you can do what I do... a couple turns of plumber's teflon tape and a drop of BLUE Loctite (and I do mean a drop applied to the tape) to hold the extender firm) then fully open the presta valve (some extenders won't come with O rings).

Carefully screw on the extender (being careful not to close the presta valve while doing so) and you're good to go.

Some caveates.

Most generic extenders will allow you to accurately fill the tube from a floor pump with a guage... but you could be SOL (at least for getting proper pressure) out on the open road. You generally have to go by feel (unless your frame pump has a guage).

Deflating can be a problem as well... I carry a small length of brake cable (soldered to keep it from unravelling) that can be stuck down the extender to actuate the valve.

If you don't use a drop of Loctite, the extender may inadvertently unscrew itself when you affix your pump. Make sure you use BLUE Loctite so you can easily remove the extender when the time comes. Just make sure you check from time to time. Also, be careful when exerting force to the extension... so's not to snap off the tube extender... or damage the tube/tube extender seat.

Also,

Some extenders either accept a presta valve core (thus functioning like a regular valve) or have their own valve built into them. In certain situations, I've found this type desirable.

Good Luck.
